Welcome to the CPHR Alberta Board of Directors Online Nomination Form

You will need to complete this form in one sitting, so please make sure you review the review the position profile and have the following prepared:

  • The four written responses on this page (Statement of Intent, Board Service, Governance Experience and Areas of Expertise) are the only written responses on the application. Please take some time to review these questions before advancing to the remainder of the form
  • Nominations require the support of two Chartered (CPHR designation holder) Members. You will need to provide these members' names, emails and phone numbers
  • Nominations also require three professional references. You will need to provide these references' names, emails and phone numbers
  • If you are selected as a Candidate, your Biography and Photo will be edited for use in the Election Package. You will need to upload these files
  • You have the option to upload your resume as part of this nomination form

 


DECLARATION: As a Chartered member of CPHR Alberta, I put forward my name for nomination and agree to serve the applicable term of office on the CPHR Alberta Board of Directors should I be elected. I attest to meeting all specified criteria outlined in the bylaws of CPHR Alberta.

CPHR Alberta By-law: 5.2 Qualifications

In order to be eligible to be elected as a Director, a person must:

  1. Be an individual who is at least 18 years of age;
  2. be a Member in good standing;
  3. reside or work in any Region approved by the Board;
  4. have skills and experience commesurate with the needs of the Board;
  5. meet any other criteria as established by the Board, including but not limited to reference checks prescribed by the Board;
  6. not be the subject of any disciplinary proceedings by the Society;
  7. not have a finding of professional misconduct, incompetence, with respect to the person in the three years preceding the date of this election;
  8. not have been convicted in or outside of Canada of an offence in connection with the promotion, formation or managment of a corporation or unincorporated entity, or of an offence involving fraud, unless such person meets and exception provided in the Act;
  9. not have the status of undischarged bankruptcy;
  10. not have been found to be incapable by any court in Canada or elsewhere; and
  11. not be an employee of the Society.
